Changes at Symphony Healthcare:

Insurance: As of October 1, 2024, Symphony Healthcare will be out of network with all commercial insurance plans, but will remain in network with Traditional Medicare Part B (First Coast – or the “red, white, and blue Medicare card”)


Primary Care Office Visits – for those of you who want to continue with our office, we will provide primary care services but our focus will be on helping patients with Thyroid, Hormone, Auto-Immune and Gut Microbiome issues. By not having to comply with insurance regulations, I can better provide a comprehensive, functional / multi- dimensional approach to your health issues. If you choose to leave the practice, we will transfer medical records electronically to a new primary care medical provider of your choice at no fee, with a signed consent form.


Only Traditional Medicare insured patients will have insurance claims filed.


Individual Self-Pay Visits –

  •  For new patients the first visit fee will range from $150 – $250 based on complexity and time.

  •  Standard Follow up visits will be $100 per visit up to 30minutes. $150 up to 45minutes. $200 up to 60minutes. $100 for an additional time up to 60 minutes
    – Visits will be scheduled as needed for your unique health issues.

  •  A “Super Bill” can be created at the end of the visit if requested for those with out of network insurance benefits – the patient can send it in for reimbursement.

  • Wholesale pricing is available for self-pay labs.

  • Weight Management Programs – Weight Management options include a holistic method using no medications, a new oral GLP-1 product, using a compound GLP-1 injection medication, or using other appetite suppressant
    medications as needed.
     
  •  Hormone Pellet Programs – Frequency of visits will be determined by your needs, typically every 3-4 months. Hormone labs can be completed using your medical insurance prior to our visits.

 

Medication Refills – Will only be done at the time of your visit to confirm the correct dose and use.

On-Line Scheduling – We will be offering on-line self-scheduling soon.

Questions will be answered within 24-48 hours.
Medications are only refilled at time of visits.

“Optimantra” is no longer functional.

“PatientFusion” is your new patient portal  – Please sign up for this secure, HIPAA- compliant portal –for email & text messaging, document-sharing & Telemedicine visits. You have access to your patient information profile to view and update as needed. You will also be able to view lab reports when completed.
