Patient Fusion gives you a real-time connection to your health information online. Take control of your health with access to your diagnosis history, medication details, immunizations and appointments.
Instructions for accessing patient portal
Step 1: Invitation Email
You will receive an email from Patient Fusion with instructions for accessing the records online.
Step 2; Sign-Up/Login
Follow the instructions in the email to Access Your Records. You will be directed to the Patient Fusion Login/Sign-Up page.
If you have never had a Patient Fusion account proceed with the Sign-Up process, otherwise you should Login with your previously created credentials and proceed to Step 4.
Step 3: Confirm your Email
After completing the Sign-Up process, you will receive a confirmation in your email. Click confirm in the email and proceed to login with the credentials you created.
Step 4: Verify your Enrollment
Login to Patient Fusion, and follow the registration steps detailed below. You can register by using one of the following methods:
- Using the temporary PIN (For Symphony Healthcare, please use option 2)
- Using your phone number recorded by the doctor
Link your health information to other applications
Once you’ve successfully created a Patient Fusion account, you can use your Patient Fusion login credentials to authenticate to any third-party application that has registered with Practice Fusion and connected to our public APIs.

Request for Medical Records.
If you are requesting Symphony Healthcare Inc. or Synergy Health Consulting to either release or obtain medical records on your behalf, we require this form to be completed and submitted to our office to process your request. You are also able to fax this to our office at 352-629-7833.
RX Requests
If you are a current patient and would like to request medication refills, please read below.
Please note our prescription policies:
- We do not fill antibiotics without performing a proper examination. Please contact us to schedule an appointment.
- For any medications that you take daily (and for chronic conditions such as blood pressure, cholesterol, diabetes, depression, etc.), our policy is that you are seen in the office every 3-6 months based on need. If you have not been seen in the past 6 months, please contact our office to schedule an appointment. If it has not been more than 6 months since your last office visit, we typically will refill a 2-4 week supply to allow you time to schedule your appointment and be seen.
- Our office does NOT prescribe controlled medications. Please contact us to discuss other alternatives.
